Application for Veteran Enshrinement

All US Military Veterans honorably discharged, living or deceased, from anywhere or any branch may be engraved on the Wall of Honor and be featured on the website.

To complete the enshrinement process:

Complete known information on the form below; Click "Submit".
Mail the following items to WVMFF, PO Box 83, Presque Isle, WI 54557. We can return your photo if you so request.
- Evidence of Honorable Discharge or DD-214
- Photo of your hero (military photo preferred)
- Short biography for the website
- Check for $300 payable to WVMFF
